Description

Beanstalk Academy's Early Intervention division is looking to hire a full-time Case Manager.
The Case Manager is responsible for the case management, and maintenance of client files and program paperwork for Home-based services are provided to clients enrolled in the Early Intervention Program.

Responsibilities:

  1. Ensure that all cases with corresponding services commence within 14-days of the start date and notify corresponding parties per EI/Agency regulations and guidelines

  2. Enter all data entries for all new/ongoing cases

  3. Maintain updated documentation as per service provision guidelines and required by NYC EIP.

  4. Provide timely follow-up with the Ongoing Service Coordinator regarding the start of care date and the ongoing status of each case.

  5. Request/obtain required paperwork for all initial and ongoing service provision cases.

  6. Forward necessary paperwork including revised or renewed documents to each assigned provider and client.

  7. Use available system reports to monitor Service Plans, and ensure compliance according to Agency and EIP regulations.

  8. Enter Data and update all required fields promptly and efficiently into ProviderSoft.

  9. Track, report, and document gaps in services, vacations, extended leave, etc.

  10. Weekly monitor attendance for all service provision cases as per approved service

    Qualifications:

    1. One (1) year of experience working within an Early Intervention or similar program providing administrative services, direct care, or case coordination
    2. Computer literate in Microsoft Office and Google Chrome

    3. An Associates degree in a Human Service (preferred)
